Since the release of the U.S. Institute of Medicine (IOM) report “To Err is Human”, which said that greater than 1 million preventable errors in all likelihood occur every year within the U.S., and of these between 44,000 and 98,000 results in death, the profession has continued to search for proven methods to preserve/improve patient safety and reduce medical errors. Medical errors are often attributed less to incompetence and more to communication barriers, ineffective teamwork, and systemic failures. Physicians need to be educated about contemporary approaches to ensure that the culture of their workplaces supports effective teamwork, respectful communication, and cohesive systems.
